NOTE: If your district has an agreement with a third-party administrator (TPA) to administer their 403(b) program, you are required to complete additional forms from the TPA to process most transactions including withdrawals, change vendors, name and address changes, beneficiary changes, and contribution changes. Please contact the TPA or your payroll office for the additional forms.
