 An enrollment booklet includes everything an employee needs to enroll in a 403(b) or IRA retirement savings account with WEA Member Benefits. 

Salary Reduction Authorization Form 

This form provides employee authorization to have 403(b) contributions deducted from their paychecks. 

We offer a standard 403(b) salary reduction authorization form for districts to use. However, if your district uses a customized SRA, make sure we have a current copy for our files to provide employees who call to enroll in your plan.

IRA contributions should be made using the IRA contribution form.

ACH payments

Remitting your employee's 403(b) contributions electronically credits your employee's accounts more quickly, reconciles your accounts faster, and provides you with an electronic record where you can track the successful delivery of the funds.

There are three options available:

  1. Trust Advantage™: Member Benefits’ Trust Advantage program provides districts with an opportunity to enhance their employee benefits without impacting their bottom line.
  2. ACH pull: Log onto your plan sponsor web site, submit your 403(b) remittance file, and elect to have us pull your funds. You will need to complete a School District ACH Authorization form prior to this option being available in your plan sponsor Web site.
  3. ACH push: Submit your 403(b) remittance and then send the funds via ACH to us.

Contribution Limit Calculations

Participants who have 15 years of service with your district may be eligible to contribute an additional amount to their 403(b) account. A Contribution Limit Calculation form (pdf) should be completed for these individuals. If you have questions about a participant’s eligibility or need a calculation completed for a participant, contact us at 1-800-279-4030, ext. 8567.

Employee status templates

For districts who use WEA Member Benefits to administer employer 403(b) vesting contribution benefits. 

As part of the administration of vesting benefits in your 403(b) plan, we need to make sure we have the most up-to-date information about your employees' status each year. Please use the following templates:

>> For employees who have left the district (separated service)

>> For NEW employees that are eligible for the plan

>> For all current employees eligible for the vesting contribution

>> For all current employees who have reached the minimum age requirement to be 100% vested in the vesting contributions
